Heat Illness Reporting Form

If you experience heat-related symptoms on the job, document it immediately — while the details are fresh. This form takes about two minutes to complete on your phone.

Using the OSHA Heat Safety Tool

Download the OSHA Heat Safety Tool and use it in conjunction with the Unionsafe141 Heat Illness Tracker to help identify and report heat-related hazards.

  • Check the daily heat index before starting work.

  • Follow the recommended precautions based on the current risk level.

  • Stay hydrated, take breaks when needed, and report any signs of heat illness immediately.

  • Complete the IAM Heat Reporting Form whenever a heat-related concern or incident occurs.
